What the research says
A quarter of older adults, by the most reliable measure
A 2023 systematic review and meta-analysis screened 11,077 records and pooled 61 studies covering 22,398 non-hospitalised participants aged 65 and over. Using directly measured osmolality above 300 mOsm/kg — the most reliable measure — it estimated that 24% were dehydrated.

Illness raises the odds further
In subgroup analyses, older adults with more pre-existing illnesses showed 37% prevalence against 15% in others, with a non-significant suggestion that those with renal impairment were higher again at 42%. Notably, there were no clear differences by age, sex, functional, cognitive or diabetic status.

It is hard to self-detect
Intake is frequently inadequate: nursing homes have reported inadequate fluid intake among 50–90% of residents. A 2020 study reported that 60% of older participants overestimated how much fluid loss it takes for moderate-to-severe symptoms to appear, and most could not identify whether they were dehydrated.

Dehydration in older adults can be serious. If someone is confused, very drowsy or unable to keep fluids down, seek medical help immediately.
